I had the same problem with 6.5.5 Beta (MIUI 7.4), but 7.5.4.0 stable seems to be slightly better (but still unusable for VoIP calls/video). The issue is observed when on speaker or earphones. Your mouth has to be really close to the mic, otherwise the other party cannot hear a thing.
