All phones that are flashed to something else then China ROM, are flashed via EDL mode, since no unlock is needed.

Well it's obviously not a phone or software issue, based it doesn't occur on ALL carriers. Mine can drop to E sometimes in regions where's no signal (and I didn't have anything more with other Android phones), then change to H, then 4G.

That's no bug, the Documents app is the one who grants permissions for writing on external storage. Maybe it's annoying to enable Documents, but once you do this, it remembers the grant. Anyone on 3S who flashed this ? What's the Android Security date ?
