In need of some serious help

Recently my phone (HTC Incredible) has been going crazy and just not working. I'm fed up with trying to fix it, and I'm at the point where I want to totally unroot my phone and bring it back to the stock OS, but I can't get it to work for the life of me. Can anyone here help me out, please...
