I Can´t Unlock The Bootloader For My Mi 5s

Hello. My old device was Mi 4c and I unlock with some problems in the Ulock page because didn´t show the form page, it was because I didn´t log in , but now with my new Mi 5s I tried to do the same steps, but allways show the download page, its mean don´t show the form page to do the unlock request. Anybody can help me?

You don't need to apply again for unlock.
One device per 30 days.
I applied once and unlocked 3 devices. Just associate the device with unlock in developer tools and unlock
