Help: Error Filling Mi Unlock Form

Hi all,

I've been trying to apply for permission to unlock my recently-purchased Xiaomi Mi 5X. Whenever I try ussing the form, it keeps asking me to "input correct name", although I'm sure it's the right one. Can anyone please help me figure out what's wrong?

You have to use the same "name" or nick you have for your Mi account, not your real name.
